Late Policy

Flourish encourages prompt arrival to each appointment. Please call the front desk 614.545.8300 ext. 1 to let us know you are running late, or email your therapist directly. If you are fewer than 10 minutes late to your appointment your therapist will see you until the scheduled therapy end time.

24-hour Cancellation Policy

$30 fee will be charged to your account for preplanned cancellations of therapy sessions (e.g., vacation, conflicting appointments etc.) not given within 24 hours of your appointment time. For unforeseen events such as illness, we ask that you call to cancel your appointment at least 4-hours before your scheduled time. Very short notice sick cancels may still result in a fee.

$100 fee will be charged to your account for evaluation cancellations less than 24 hours before the appointment time.
Unforeseen circumstances are anticipated and will be dealt with on a case-by-case basis.

 

Attendance Policy

Flourish requires an attendance policy of 80% to remain on the schedule. If your attendance dips below 80% you will be warned. If attendance remains under 80%, Flourish Integrated Therapy, LLC reserves the right to terminate services, relinquish your appointment time, and/or refer you to another service provider. In the case of two successive no-shows or three no-shows in a 60 day period, Flourish Integrated Therapy, LLC reserves the right to terminate services, relinquish your appointment time, and/or refer you to another service provider.

Sick Policy

Flourish follows all CDC and Columbus Public Health guidelines. Please keep your child home if he/she is exhibiting any of the following symptoms: Temperature of 100.4* or higher, fever/chills, persistent cough, shortness of breath, unexplained fatigue/body aches, loss of taste and/or smell, sore throat, vomiting, diarrhea. If there has been a past illness, your child must be symptom and fever-reducing medication free to return to therapy. For unforeseen events such as illness, we ask that you call to cancel your appointment at least 4-hours before your scheduled time. Very short notice sick cancels may still result in a fee.

 Policies

We travel during the summer months. Will my therapy spot be held for me when I return in the fall?

No. We cannot hold therapy appointment times for longer than two weeks.

My child has been sick for a few weeks. Am I at risk for losing my appointment time?

Flourish encourages families to stay home when sick. We will work with families on an individual basis when situations like these arise.

Forms

Do I have to have a Credit Card on file?

Yes. All clients are required to have an active credit card on file. We send out billing statements twice per month (around the 1st and the 15th). You will have 2-3 business days to review the statement prior to the card on file being charged. If you have an HSA or FSA card, you must also have a secondary credit card on file.
