Getting Started

Please fill out the contact form so we can gather client information.

Please email a front and back copy of your insurance card to office@flourishohio.com or fax it to 614-754-5230. Be sure to include the patient’s name, as well as the policy holder’s name and date of birth.

If you have an evaluation from another facility, please email this to office@flourishohio.com or fax it to 614-754-5230. We must review all outside evaluations. All evaluations must be completed and dated within the last 12 months.

Once we have steps 1-3 completed we will call you to schedule appointments. It may take 2-4 business days to get your paperwork reviewed. Please feel free to contact us to ensure we received the intake information. Thank you for your cooperation!

Once your appointment is scheduled, we will send electronic intake paperwork. This paperwork must be completed 24-48 hours prior to your first appointment. Failure to do so may result in cancellation of your appointment.

Once your appointment is scheduled, It is strongly recommended that you contact your insurance company in order to ensure that this form of therapy is a covered benefit that will be paid for your child's specific condition/diagnosis. Diagnosis and CPT codes may change during the course of therapy. Flourish Integrated Therapy, LLC will work with your insurance company to make every attempt to have these services paid, but your involvement is very important in this process. Making contact with your insurance is the first step in getting these services paid. Please remember that the insurance company cannot guarantee payment until they process your claim.
