Becoming a New Patient
We are so excited for you to become a part of the Flourish Family. Our team provides neurodivergent-affirming treatment for children of all ages and levels of support.
Please follow the steps below to get started with our therapy services.
Please fill out this form so we can gather client information. All of our scheduling is done via Ocean Friends.
Once your appointment is scheduled, It is strongly recommended that you contact your insurance company in order to ensure that this form of therapy is a covered benefit that will be paid for your child's specific condition/diagnosis. Diagnosis and CPT codes may change during the course of therapy. Flourish Integrated Therapy, LLC will work with your insurance company to make every attempt to have these services paid, but your involvement is very important in this process. Making contact with your insurance is the first step in getting these services paid. Please remember that the insurance company cannot guarantee payment until they process your claim.

What is my diagnosis code?
Please refer to your evaluation report for the diagnosis code. This is at the top of all of our reports.
My insurance company reported that speech and OT is covered. Why am I being charged?
Please call your insurance company to understand your deductible, co-pay, co-insurance amounts. Just because treatment is covered, does not mean that you will not have a patient responsibility.
